I saw one person say they broke vengeance oath by not killing the evil hag. Vengeance oath is to kill evil things. Try not killing something evil you encounter. Or you could respec to Oath of devotion and kill one random person who's not evil. If you do the quest involving and , you are given the choice to torture or forgive the latter.In Act 3, a consistent way for me to break the Oath of Vengeance is to make a deal with Gortash at his coronation. Breaks it every time. Another way is in the goblin camp in act 1 if you torture the prisoner inside the camp. Either on purpose or by failing any of the checks. Don't kill random people,stick to your word,don't side with the badguys. You cannot be a paladin and a murderhobo unless youre an oathbreaker.
